Wisconsin Immunization Registry (WIRS) Opt Out Form

If your child's birth certificate is registered in a county in the state of Wisconsin and you are not up to date on Big Pharma's schedule for your child's vaccinations, you are likely to be contacted by your local health department with pharmaceutical-sponsored forms like
these asking you to catch your child up... unless you submit a Wisconsin Immunization Registry System (WIRS) opt out form to the state. Doing this inactivates your child's name in the state database, and the bothersome mail from your local health department should end.
The State of Wisconsin Department of Health Services has myriad forms available in their online forms library. Having contacted them to ask why they don't have the WIRS opt out form available online, they explained that they don't have to offer the exclusion program at all per HIPPA rules, and that they want parents to have to call in so they have the chance to educate them.

here. Note the last sentence in the Opt Out Form. We suggest you add the two phrases in bold italics before printing your form: I understand
the State alleges that removing this information may result in the inability to properly assess the immunization status of the child named above and
the State alleges could therefore leave him or her susceptible to a vaccine preventable disease or result in unnecessary immunizations.
